Summary:

Epilepsy is among the most common neurological disorders in the world, affecting one in every 100-200 people. In an effort to harness progress made in mapping the human genome, a team ofU.S. scientists in epilepsy centers across the country have received a grant of $15 million dollars to study the complex genetic factors that underlie some of the most common forms of epilepsy. To meet the mission of the project, a technology framework that allows researchers to gather, correlate and analyze a huge number of variables must be fielded in a cost-effective manner, while complying with strict regulations and privacy policies. To meet this challenge, the EPGP turned to Microsoft and Chennai, India-based TAKE Solutions to develop a robust but flexible web-based infrastructure. The team has very effectively helped to advance medical science in the area of epilepsy research by using advanced technologies to streamline clinical operations and remove IT from the critical path of discovering new therapies and cures.

UC San Francisco is the 4th ranked Schoolof Medicine in the United States and is part of the University of California System, the largest Higher Education institution in the country.

Business Problem:

Human genome mapping is a relatively young science. As a result, the information systems to support these types of research trials are relatively immature. However, having received a major grant, it was important to rapidly deploy the underlying technologies that would allow the project to move forward smoothly to ensure that early and ongoing success of this project.

From that perspective, the natural place to start looking for the right technology was from solutions already offered by providers in the life-sciences sector. An early review of technologies in the field, however, did not prove to be promising.

“We were not able to find a vendor of packaged solutions that we felt suitably mapped to our requirements. Most solutions on the market are regulatory-compliance oriented and are designed for the pharmaceutical industry’s needs. They are not able to handle the very heavy and rigorous demands that we have in terms of things like collecting information for data-intensive surveys, for instance,” says Michael Williams, EPGP CIO.

Solution:

With existing packaged application providers deemed untenable, Williams determined that the project needed solid generic workflow control and document management technologies on top of which new applications could be rapidly and cost-effectively developed. He was specifically interested in technologies that could be managed in a web-based environment. After an exhaustive review of technologies, vendors and industry best practices, Williams and his team decided to standardize on Microsoft technology to provide the basic functionality.

  • Microsoft Office Sharepoint Server (MOSS) was selected for its comprehensive document management and data aggregation capabilities.
  • ASP.NET was chosen for its flexibility in delivering customized web-based functionality.
  • Microsoft Windows was chosen as the operating system of choice.
  • Microsoft Information Security Accelerator (ISA) server was selected to provision the firewall security.
  • Microsoft SQL 2005 was chosen as the database application for the data repository.

With the enterprise infrastructure elements in place, Chennai, India-based TAKE Solutions was tapped to develop the specific applications that would drive the various mission critical processes of the project.

Result:

The results have been impressive. “We’ve gone through the development of the entire Epilepsy Phenome/Genome Project in 12 months. And we have modeled and tested each key component from cradle to grave,” says Williams.

The project is currently underway, with the public EPGP website now available to disseminate information to the general public, individuals and families interested in participating in the clinical study. It is also a focal point for other professionals interested in the EPGP endeavor.

For those directly involved in EPGP initiative, the website serves as the starting point for accessing the EPGP intranet site, which contains the document management system, phenotype data collection applications and specimen management programs. The site is in production, and has accomplished the following:

  • The EPGP Application Suite has successfully been used for 425+ recruits and 185+ Enrolled Participants at 13 clinical centers.
  • All required sites are currently using the EPGP Application Suite based on a unified architecture for:
    1. Recruitment
    2. Patient Visit Tracking / Schedule of Events
    3. Electronic Case Report Forms, including Phenotypic Instruments
    4. Specimen Tracking to Coriell for storage
    5. EEG and MRI review
    6. sFTP for core data file uploads
    7. EPGP BioMed Data Repository
    8. Public Website
    9. Portal and Document Management
  • Support requests at all sites have been low, and self-reported site coordinator satisfaction with the tool suite is overwhelmingly positive.
  • Biomedical Informatics and Information Technology budgets, while slightly over from a cash-flow perspective in the first year, are well on track to meet projections over the 5 years of the EPGP Project funding.
